     The ATF have issued a new Form 4473 that you must fill out when ever you buy a gun of any type. This is a copy of the form and as always, it is always subject to change without notice.
     This is not the full form, this is only the section that you fill out. The back of these three pages have more information about how to fill in the form, the law, definitions and other information.

New ATF Form 4473 - Firearms Transaction Record - Requirement for Federal Firearms Licensees

This serves to remind all Federal Firearms Licensees (FFL) to begin using the new ATF Form 4473 on Monday, October 17, 2005. At this time, every FFL should be in receipt of the new forms (OMB No. 1140-0020). The new forms are required to be used beginning October 17, 2005, as previous versions will be obsolete. If any FFL is not in receipt of new ATF Forms 4473, he or she should immediately contact their local ATF office. If any FFL has received new forms but wish to place an additional order, please contact the Distribution Center at (703) 455-7801.
